Notice of Privacy Practices

You may pre-register with our office by filling out our secure online Patient Registration Form. After you have completed the form, please make sure to press the “Complete and Send” button at the bottom to automatically send us your information. The security and privacy of your personal data is one of our primary concerns and we have taken every precaution to protect it.

If you choose not to register online or experience any difficulties using our online forms, you may download and complete the Registration Forms by hand and bring them to your first appointment.  Please click on the link to access forms.  

If you would like to complete the forms in our office, please arrive 30 minutes before your scheduled appointment.

Our office hours are from Monday through Thursday: 8am-5pm and Fridays: 8am-3pm.
