First Visit:
Your initial visit is a crucial step in the treatment planning process. Dr. Ismaili will review your health history, perform an examination, explain your diagnosis, and create a treatment plan. Your consultation visit will conclude with a brief visit with one of our financial coordinators. They will review the proposed treatment plan and the associated fees and schedule a convenient time to schedule your surgery.
Occasionally, surgery can be performed the same day as the consultation, should you choose to and are prepared. (You must have an escort to drive you home and please review the instructions for Anesthesia below).
Please assist us by providing the following information at the time of your first visit:
• Referral form or letter from your referring dentist/doctor
• X-rays that have been taken by your referring dentist
• An insurance referral if you have a dental HMO policy
• A list of all medications you are presently taking
• Picture ID
• Your dental and/or medical insurance cards
• Your completed Patient Information and Health History packet
Finally, please be prompt for your appointment time. You should expect your consultation to take approximately one hour.
Important: All patients under 18 years of age must be accompanied by a parent or legal guardian for consultation and surgery. |

Financial Policy:
We make every effort to provide the finest surgical care at the most reasonable cost to our patients, therefore, payment is due at the time service is rendered. You may choose from one of the following options to pay for your treatment:
- Cash or Check
- Visa, MasterCard, Discover, American Express
- CareCredit – Depending on amount to be financed, you may be eligible for a 6 month interest-free payment plan as well as other low monthly payment options. If you are interested in this option, you may visit the CareCredit website at www.carecredit.com.
Please remember you are fully responsible for all fees charged by this office regardless of insurance coverage. |

Insurance:
As a courtesy to our patients, our office files all insurance claims and will assist you in obtaining your maximum level of benefits from your insurance company. As some of the services we provide in our office are deemed both medical and dental by many insurance companies, it is important that you provide our office with detailed information regarding all insurance policies in place prior to the time services are rendered. Additionally, if your insurance company requires that our services are pre-authorized, we will work with you to obtain such authorization in advance.
Prior to your appointment, we will obtain your insurance benefits based upon information that you provide us. Depending on your insurance, on the day of your appointment, we will collect the amount that we estimate to be your responsibility. Once insurance has processed your claim(s), you will receive a statement for any balances still due. Most Insurance companies will respond to our request for payment within four to six weeks. However, after 45 days of filing your claim, if the insurance company has not issued payment for your claim, the balance on your account will become solely your responsibility. We kindly ask that you remit additional payments promptly.

Special Instructions for Patients Receiving Intravenous Anesthesia
Following are instructions to assist you in preparing for your surgical appointment:

- Do not eat, or drink (not even water) 6 hours prior to your surgery. Intravenous anesthesia is very safe if the patient’s stomach is empty.
- You must be accompanied by a responsible adult the day of surgery. This individual must wait for you in the reception area during the procedure and must drive you home after surgery. Taxicab rides are not allowed.
- Your surgery will be rescheduled if you eat, drink, or come to the office without someone to drive you home and stay with you at home.
- Do Not drive or operate dangerous equipment for at least 24 hours after anesthesia is given.
- Please bring a list of all medication you are taking with you.
- Leave all valuables at home. Purses, jewelry, wrist or pocket watches, phones, wallets, facial or oral piercing devices, and any removable dental appliances are not allowed in the operating room. If you wear glasses, please leave them with your escort.
- Your surgery will take about an hour. In some circumstances it may take longer You will recover in the observation area for 15 to 20 minutes. You will need a responsible adult to stay with you at home for several hours after surgery.
- Please wear short sleeves or loose comfortable clothing.
- Children under 18 years of age must be accompanied by a parent or guardian on the day of surgery. Children may not be left unattended.
